Change for Life

Program Description

To help African Descendents and Latinos prevent diabetes and high blood pressure. The program includes group support activities and an easy-to-use workbook in English and Spanish. Groups are led by trained community members

What is the Change for Life Program?
Change for Life provides program participants with the knowledge, skills and social support they need to make health behavior change. Some participants focus on diet, others on exercise, still others on managing chronic disease like asthma or diabetes. Most importantly, they are in control of what they do and how they do it and the program provides them structure, support and motivation.

Some UNIQUE features of the program are:
  • It's PERSONAL. Participants decide what to change and set their own goals
  • It's PROVEN. participants quickly adopt new behaviors and gain measurable results like weight loss and blood sugar level reduction
  • It's COST EFFECTIVE. The group approach is significantly less costly than individual counseling or memberships
  • It fits DIVERSE PARTTICIPANTS. Change for Life fits for people of all literacy levels and the Cambia tu Vida version is for Spanish-speaking participants
  • It's FRIENDLY. Programs take place in familiar environments and are led by familiar facilitators from the sponsoring organization
  • It's RESULTS ENDURE. Participants learn skills that help them maintain positive changes and make new ones
