Our Mission
The mission of NH REACH
2010 Initiative is to improve the health status of African Descendent
and Latino communities in New Hampshire, through research, education,
and outreach, with a focus on diabetes and hypertension.

What does REACH mean?
The acronym REACH stands for Racial and Ethnic Approaches to Community
Health.
REACH 2010 is an initiative that was formed out of the Centers for Disease
Control and Prevention (CDC) Healthy People 2010 Initiative.
Healthy People 2010 was designed to achieve two overarching goals: (1)
To increase quality and years of healthy life; and (2) To eliminate health
disparities.
NH REACH 2010 Initiative is proud to be a part of helping the CDC to
achieve these national health objectives.
Initiative Goals
The six goals of NH REACH 2010 Initiative are designed to implement culturally
effective programs for addressing health disparities in diabetes and hypertension
within the African Descendent and Latino populations while collecting
and utilizing data to advocate for health equity for all of New Hampshire.
- Strengthen the coalition's capacity to address health disparities
in NH's racial and ethnic communities.
- Develop systems for outreach in African Descendent and Latino communities.
- Develop and implement programs for screening and health education
in African Descendent and Latino communities.
- Develop programs for referral to primary care based on identified
barriers to medical care.
- Develop programs to assist health care organizations in providing
culturally effective care.
- Establish data systems that will assist program planning and evaluation
and inform state-wide policy efforts addressing the needs of racial
and ethnic populations.
